Accident Reporting - School Visitors & Contractors

School Visitors:

All visitor injuries must be reported to the school admin office as soon as possible. The school administration team member will provide visitors with a accident reporting package.  Visitors must complete the MSBA accident report and return it to the secretary before leaving.  The school secretary will enter the accident report online.

  • School visitor accidents must be reported electronically via the  MSBA Accident Reporting website using the Non- Student Accident Incident Online Report Form.  
  • The school safety and health committee employer and worker co-chairs must initiate an accident investigation, to determine the cause of the incident and implement control measures to prevent the incident from happening again.   
  • Severe cases shall be reported immediately to the Division’s Safety Officer and will also be reported to the Secretary Treasurer and to the SAT Link.  


Contractors Working on School Property:

  • All accidents must be reported to the school division Safety Officer.  
  • An investigation report must be completed for all serious incidents by the contractor and a copy forwarded to the Division’s Safety Officer. It is the contractors responsibiltity to report all serious incidents to the Workplace Safety & Health Division.  Note: serious incidents will also be reported by the school division's safety officer.
  • Severe cases will be reported immediately to the Secretary Treasurer and to the Assistant Superintendent – Divisional Support Services.
